Immunization Information

To help protect the school community from potential outbreaks and illness, the Washington State Department of Health requires students to show proof of the required vaccinations or a current WA State exemption for school attendance.

The Auburn School District (ASD) uses the Washington Immunization Information System(WAIIS), a lifetime immunization record, to monitor immunization compliance. WAIIS requires the ASD to only accept medically verified immunization records for all students.  Medially verified records must have your child's name, your child's date of birth, and a unique health care provider or clinic stamp or another form of written healthcare provider documentation, such as a provider signature.

Examples of medically verified records include one or more of the following:

• Official immigration immunization records
• The immunization record (Certificate of Immunization Status (CIS)) printed from WAIIS or another state's system
• A written immunization record from a health care provider or cliic with a healthcare provider's signature
• The immunization record printed from a healthcare provider, clinic, or hospital's Electronic Health Record
• A CIS printed from MyIR Mobile 
  • MyIR is a Washington State authorized record access system for immunization records. You will need to set up an account to access your student's information. You can also call or e-mail the DOH office at WAIISRecords@doh.wa.gov or 1-866-397-0337.  

Students will NOT be able to start school if they are out of compliance.  Students must be in compliance and have submitted medically verified records on or before the first day of school to be allowed to attend. If a student is attending school and found to be out of compliance during the school year, they may be excluded until they are back in compliance with the state requirements.

Please sign the Certificate of Immunization Status form to give us permission to update the state system, and turn it in to your child's school with their medically verified immunization records.

Exemptions  

Washington State Law allows parents or guardians to exempt their child from school immunization requirements in some cases. Exemptions may be claimed for personal/philosophical, religious or medical reasons. Measles, mumps, and rubella (MMR) may not be exempted for personal/philosophical reasons.

If you decide to request an exemption for one or more of the immunization requirements, you must provide the school with a completed Certificate of Exemption (COE) which can be found at the above "immunization requirements" link as well.  This form is available in 16 different languages.  If you have additional questions, please contact your school nurse.

Conditional Status

Students can be granted conditional entry status if they are making progress towards completing their immunization requirements and have proof of an upcoming appointment within a reasonable time frame that has been provided to the school.  If additional immunizations are needed, a student could start school in conditional status and remain until the appointment date provided.  The student must present updated immunization records from the provider upon their return to school after the appointment.  If an appointment is missed, the student will be considered out of conditional status and may be excluded until all required immunizations are complete.
